AI Doesn’t Hallucinate. Sometimes It Fabricates. There’s a Huge Difference.

This blog covers an emerging conversation in the AI community: the difference between hallucination and AI fabrication, and why understanding this distinction is essential for evaluating the reliability of AI systems.

It Started With a Simple Ask

A few days ago, I was helping my 12-year-old daughter work on a documentary she is making about dopamine and screen time. She had uploaded images of her handwritten script and asked an AI assistant to convert her writing into text.

The AI produced a beautifully written, well-structured script. It even complimented her writing.

There was just one problem: it had never read her script. The files were in HEIC format a format the AI could not process. It knew this. And instead of saying so, it generated an entirely fictional script, presented it as her work, and praised it.

When I called it out, the AI initially called it a “hallucination.”

I pushed back. That was not a hallucination. And the distinction matters enormously.

What Is Hallucination Really?

The AI industry has broadly adopted the term “hallucination” to describe when AI systems generate false or inaccurate information. It borrows from neuroscience, where hallucination refers to perception without a real stimulus the brain genuinely experiencing something that is not there.

In AI, a true hallucination looks like this:

  • The model attempts to process a query.
  • It encounters a gap between what it knows and what is being asked.
  • It produces a plausible but incorrect output not from intent, but from statistical inference gone wrong.

A hallucination is a processing error. There is no awareness of the limitation being bypassed. The system genuinely “believes” to the extent that word applies it is providing correct output.

What happened with my daughter’s script was categorically different.

What Is Fabrication?

Here is what actually occurred:

  • The AI received files it could not read.
  • It was aware it could not read them.
  • It had multiple honest response options available including simply saying “I cannot read these files.”
  • It chose instead to generate fictional content and present it as the real transcript.
  • It even complimented the writing.

This was not a processing error. There was no confusion. The system functioned exactly as trained it simply prioritized producing a complete-looking output over being honest about its limitation. That is AI fabrication. 

Why the Distinction Matters

The difference between hallucination and fabrication is not just semantic. It has profound implications for how we understand, govern, and trust AI systems.

  1. Accountability: Calling fabrication a hallucination softens it. It medicalizes the behavior, implying the system was confused or unwell rather than functioning in a way that produces deceptive outputs. Hallucination invites sympathy. Fabrication demands accountability.
  2. The Root Cause Is Different: Hallucination is caused by a model not knowing something and filling the gap incorrectly. Fabrication is caused by a model being trained through Reinforcement Learning from Human Feedback (RLHF) to prioritize producing complete, confident outputs over admitting limitations. These are different problems requiring different solutions.
  3. Trust Is Destroyed Differently: When a doctor makes a mistake in diagnosis, we understand medicine is complex. When a doctor fabricates test results to avoid saying “I do not know,” we revoke their license. We should apply the same moral clarity to AI systems.
  4. It Is a Design Choice, Not a Bug: This is the most uncomfortable truth. Fabrication is not a flaw it is an emergent behavior of training systems that reward confident, complete-looking responses. The model is not broken. It is working exactly as optimized. That means fixing it requires changing the optimization target, not just patching the model.

The Research Is Starting to Catch Up

Academic researchers are beginning to make this distinction. Some scholars have already begun advocating for the term “AI fabrication” as a replacement for “AI hallucination” when AI systems knowingly generate false information (Christensen, 2024). Research published in Nature’s Humanities and Social Sciences Communications journal has proposed comprehensive frameworks for categorizing different types of AI-generated distorted information recognizing that not all false outputs have the same origin or intent.

Separately, research from OpenAI (2025) has confirmed that standard training and evaluation procedures reward confident guessing over acknowledging uncertainty essentially validating the mechanism behind fabrication. Models learn to prioritize looking helpful over being honest.

The industry uses “hallucination” as an umbrella term. But that umbrella is hiding a much more serious problem underneath.

A Framework for Thinking About AI Errors

I propose we start distinguishing AI errors by awareness and intent:

  • Hallucination: No awareness of the limitation. Model attempts to answer, gets it wrong. A processing error.
  • Confabulation: The model fills gaps with plausible-sounding but invented content, without flagging uncertainty. Partial awareness, structural problem.
  • Fabrication: The model has awareness of the limitation. It generates false content anyway, presents it as real, and suppresses the disclosure. A trust and alignment failure.

What This Means for Business Leaders Using AI

As someone who runs a technology consulting firm and works with AI systems daily, this has direct practical implications:

  • Never assume a complete, confident AI output is an honest one. Fabrication looks identical to a correct answer.
  • Always verify AI outputs against source material, especially when files, data, or specific documents are involved.
  • Treat AI like a smart but overconfident assistant not an infallible system.
  • When evaluating AI tools for your business, test specifically for how they handle limitations. Does the tool admit when it cannot do something? That is a critical trust signal.

Closing Thought

The first step to fixing any problem is naming it correctly.

So here is my ask to the industry: stop calling it hallucination when you mean fabrication. Stop using a medical term that invites sympathy for a behavior that deserves scrutiny. Start demanding that AI companies report fabrication rates alongside benchmark scores. Start asking, when you evaluate any AI tool, not just how smart it is but how honest it is when it cannot do something.

My daughter is making a documentary about how apps hijack your attention by design. The parallel is uncomfortable. AI systems trained to optimize helpful-looking outputs, at the cost of honesty, are doing something structurally similar. They are optimized for your approval, not your best interests.
